Description

This form must be complete to obtain payment. Scan of receipts must include the name of vendor, date, description of work, and total amount due. Incomplete scans will result in rejected requests. (A pdf copy of the receipt is best. If you do not have access to a scanner you can bring the invoice to church, however the payment request will not be processed until the invoice is received.
